ビタミンDは骨の健康に関わる栄養素として知られていますが、がんとの関係についても長年研究が続けられてきました。これまでの研究では、血中のビタミンD濃度が高い男性ほど前立腺がんと診断されるリスクが高いという報告もあり、ビタミンDと前立腺がんの関係は単純ではないことが示唆されています。しかし、すでに前立腺がんと診断された人において、診断前のビタミンD濃度がその後の生存率とどう関係するのかについては、これまで十分なデータがありませんでした。今回、複数の国の前向きコホート研究を統合した大規模な解析により、この点を検証した研究がBMCメディシン誌に掲載予定です。

研究でわかったこと

この研究では、13件の前向きコホート研究のデータを統合し、前立腺がんと診断された12,635人を対象に、診断前に採取された血液中の25-ヒドロキシビタミンD[25(OH)D]濃度(ビタミンDの状態を示す主要な指標)と、その後の前立腺がん特異的死亡(前立腺がんが原因で亡くなること)およびその他の原因による死亡との関連を調べました。解析にはCox比例ハザードモデルという統計手法が用いられ、複数の要因を調整した上でハザード比(HR、リスクの相対的な大きさを示す値)が算出されています。

解析対象となった3,445件の死亡のうち、1,316件(38.2%)が前立腺がんによるものでした。十分に調整したモデルでは、診断前の血中25(OH)D濃度が高い男性は、前立腺がん特異的死亡のリスクが低い傾向にあることが示されました(80パーセンタイル分の増加あたりのHR=0.75、95%信頼区間:0.64-0.88、傾向性のP値<0.001)。また、前立腺がん以外の原因による死亡についても、やや低いリスクが認められました(HR=0.85、95%信頼区間:0.75-0.96、傾向性のP値=0.009)。さらに、がんのステージやグレード(悪性度)の情報が完全にそろっているケースに限定した解析(5,078例、うち前立腺がん死761件)でも、同様に低いリスクとの関連が確認されています(HR=0.81、95%信頼区間:0.65-0.99、傾向性のP値=0.04)。

この研究の位置づけ・読むうえでの注意

この研究は、13件の前向きコホート研究を統合した大規模な解析であるという特徴がありますが、あくまで一つの研究であり、これだけで結論が確定したわけではありません。診断前の血中ビタミンD濃度と前立腺がんの生存率との間に統計的な関連が見られたことは事実として報告されていますが、これは「ビタミンDを摂取すれば生存率が上がる」という因果関係を直接証明するものではない点に注意が必要です。血中濃度の測定という観察に基づく研究であるため、生活習慣や健康状態など他の要因が影響している可能性も考えられます。

また、要旨にあるように、これまでの研究ではビタミンD濃度が高い男性ほど前立腺がんと診断されるリスクが高いという報告もあり、診断リスクと診断後の生存率とでは異なる関連が示されている点も、この分野の研究を理解するうえで押さえておきたいポイントです。

まとめ

今回の13件の前向きコホート研究を対象とした統合解析では、前立腺がんと診断される前の血中ビタミンD[25(OH)D]濃度が高い男性ほど、前立腺がん特異的死亡およびその他の原因による死亡のリスクが低いという関連が報告されました。ただし、この結果はあくまで一つの研究による知見であり、ビタミンDの摂取や補充が前立腺がんの生存率を改善するかどうかを直接示すものではありません。今後さらなる研究の蓄積が待たれます。
